eHealth Jamaica Limited Lifeline Plan Terms and Conditions.
Introduction:

Thank you for choosing the Lifeline Healthcare Plan provided by eHealth Jamaica Limited, located at Montego Bay, St James. Our plan offers exclusive health and wellness benefits tailored for individuals, families and businesses in Jamaica


Membership Plans:
We offer three affordable plans:
  • Gold Package
  • Platinum Package
  • Diamond Package

Prices are subject to change, with 30 days' notice provided to our members.


Our Commitment to You:

Maintain a network of approved doctors and medical service providers across Jamaica. Provide access to these providers according to the benefits outlined on our website through a membership card. Replace lost physical cards for a modest fee. Reserve the right to change providers and adjust rates with proper notice.


Payment and Membership:

Membership is effective upon activation, either at sign-up or upon successful payment. Payment dates are set monthly, with options for automatic payment methods preferred. Corporate accounts may arrange initial payments with prior agreement. A ten-day refund policy is in place for new members. Failure to make six consecutive monthly payments will result in membership inactivation.


Accessing Services:

Members must present their eHealth card and photo ID to access care. Providers verify eligibility through our proprietary system using the member's ID number.


Quality Assurance:

We maintain a Quality Assurance program to measure service satisfaction, with random member feedback encouraged but not obligatory.


Credit & Debit Card Payments:

Members are responsible for ensuring payment authorization and honoring payments. Failed payments may result in membership suspension or cancellation after 30 days. Reinstatement and Forbearance:

Inactive memberships have 60 days for reinstatement before requiring a new application. We reserve the right to enforce penalties outlined in the terms, but may choose not to exercise them without forfeiting our rights.


Record Keeping and Confidentiality:

Both parties agree to keep accurate payment records and treat confidential information as such. Any discrepancies must be reported within 30 days.


Termination, Assignment, and Amendment:

Termination requires 30 days' written notice. Assignment of the agreement requires prior consent. Amendments to the agreement require 30 days' notice, with the option for members to terminate if they disagree with the proposed changes.


Notices and Governing Law:

Notices will be delivered electronically or by mail to the address provided. This agreement is governed by the laws of Jamaica.
